*Discharge Coordinator - Behavioral Health*

Monday - Friday, 7:30am - 4:00pm

Position Details

What you will do in this role:

  • Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
  • Creates and maintains up to date aftercare coordination community resources
  • Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.
  • Provides resource information to patients and families as appropriate for future care needs.
  • Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients’ treatment.

Qualifications

Qualifications:

  • Bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
  • Prior experience with psychiatric and chemical dependency patients.
  • Unencumbered clinical license if eligible/needed per state regulations preferred.
  • CPR certification and Crisis Prevention Training (CPI) within 30 days.

 

Salary range: $20 - $27 per hour. We would expect those hired into this job would be paid within this hourly rate, however, certain circumstances may allow us to pay outside of this hourly rate.
